90_SB0857ham002 LRB9003267DPpkam01 1 AMENDMENT TO SENATE BILL 857 2 AMENDMENT NO. . Amend Senate Bill 857 on page 27, 3 immediately below line 13, by inserting the following: 4 "Section 17. The Illinois Dental Practice Act is amended 5 by changing Section 21 as follows: 6 (225 ILCS 25/21) (from Ch. 111, par. 2321) 7 Sec. 21. Fees. The fees for the administration and 8 enforcement of this Act, including but not limited to 9 original licensure, renewal, and restoration fees, shall be 10 set by the Department by rule. However, the fee for 11 application for renewal of a license as a dentist or 12 specialist is $75 per year and the fee for application for 13 renewal of a license as a dental hygienist is $37.50 per 14 year. The fees shall be nonrefundable.The Department is15authorized to impose the following nonrefundable fees:16(a) License as dentist or specialist.17(1) Application for temporary training or18restricted faculty license: $25.19(2) Application for initial license pursuant to20examination: $25.21(3) Applicants for any examination shall be22required to pay, either to the Department or to the-2- LRB9003267DPpkam01 1designated testing service, a fee covering the cost of2initial screening to determine eligibility and providing3the examination.4(4) Application for licensure from a person5licensed as a dentist under the laws of another state or6territory of the United States: $50.7(5) Application for renewal of a license: $50 per8year.9(6) Application for restoration of a license: $1010plus payment of all lapsed renewal fees.11(7) Application for inactive status: $10.12(8) Application for restoration from inactive13status: the current renewal fee.14(9) Application for restoration after failure to15notify Department of change of address: $15, plus payment16of all lapsed renewal fees.17(10) Application for an initial permit to18administer general anesthesia or parenteral conscious19sedation: $25.20(11) Application for renewal of a permit to21administer general anesthesia or parenteral conscious22sedation: $25 per year.23(b) License as dental hygienist.24(1) Application for initial license pursuant to25examination: $25.26(2) Applicants for any examination shall be27required to pay, either to the Department or to the28designated testing service, a fee covering the cost of29initial screening to determine eligibility and providing30the examination.31(3) Application for license from a person licensed32as a dental hygienist under the laws of another state or33territory of the United States: $50.34(4) Application for renewal of a license: $15 per-3- LRB9003267DPpkam01 1year.2(5) Application for restoration of a license: $103plus payment of all lapsed renewal fees, but not to4exceed $85.5(6) Application for inactive status: $10.6(7) Application for restoration from inactive7status: the current renewal fee.8(c) General fees.9(1) Application for a duplicate license: $20.10(2) Application for a certification of a11registrant's record for any purpose: $20.12(3) Application to have the scoring of an13examination administered by the Department reviewed and14verified: $20, plus any fees charged by the applicable15testing service.16(4) The fee for a wall certificate showing a17license shall be the actual cost of producing such18certificate.19(5) The fee for a roster of persons licensed as20dentists or dental specialists or dental hygienists in21this State shall be the actual cost of producing such a22roster.23(6) The fee for dental licensing, disciplinary or24investigative records pursuant to a subpoena is $1 per25page.26 (Source: P.A. 89-80, eff. 6-30-95; 89-116, eff. 7-7-95.)".
