Amend House Bill 3374 by replacing 3 the title with the following: 4 "AN ACT concerning transportation, amending named Acts."; 5 and 6 by replacing everything after the enacting clause with the 7 following: 8 "Section 5. The Civil Administrative Code of Illinois is 9 amended by changing Section 55.70 as follows: 10 (20 ILCS 2310/55.70) 11 Sec. 55.70. Breast and Cervical Cancer Research Fund. 12 From funds appropriated from the Breast and Cervical Cancer 13 Research Fund, the Department of Public Health shall award 14 grants to eligible physicians, hospitals, laboratories, 15 education institutions, and other organizations and persons 16 to enable organizations and persons to conduct research. For 17 the purposes of this Section, "research" includes, but is not 18 limited to, expenditures to develop and advance the 19 understanding, techniques, and modalities effective in early 20 detection, prevention, cure, screening, and treatment of 21 breast and cervical cancer and may include clinical trials. -2- LRB9009425KSgcam01 1 Moneys received for the purposes of this Section, 2 including but not limited to income tax checkoff receipts and 3 gifts, grants, and awards from private foundations, nonprofit 4 organizations, other governmental entities and persons shall 5 be deposited into the Breast and Cervical Cancer Research 6 Fund, which is hereby created as a special fund in the State 7 treasury. 8 The Department of Public Health shall create an advisory 9 committee with members from, but not limited to, the Illinois 10 Chapter of the American Cancer Society, Y-Me, the Susan G. 11 Komen Breast Cancer Foundation, and the State Board of Health 12 for the purpose of awarding research grants under this 13 Section. Members of the advisory committee shall not be 14 eligible for any financial compensation or reimbursement. 15 The Department of Public Health shall create an advisory 16 committee with members from, but not limited to, breast 17 cancer advocacy groups, medical professionals, and the State 18 Board of Health for the purpose of awarding grants funded 19 through Mammogram license plates, issued under Section 3-644 20 of the Illinois Vehicle Code, to non-profit organizations and 21 local health departments for the provision and promotion of 22 breast cancer screening and diagnostic services. Members of 23 the advisory committee shall not be eligible for any 24 financial compensation or reimbursement. 25 (Source: P.A. 88-85; 88-459; 88-670, eff. 12-2-94.) 26 Section 10. The Illinois Vehicle Code is amended by 27 adding Section 3-644 as follows: 28 (625 ILCS 5/3-644 new) 29 Sec. 3-644. Mammogram license plates. 30 (a) The Secretary, upon receipt of an application made 31 in the form prescribed by the Secretary, may issue special 32 registration plates designated as Mammogram license plates. -3- LRB9009425KSgcam01 1 The special plates issued under this Section shall be affixed 2 only to passenger vehicles of the first division and motor 3 vehicles of the second division weighing not more than 8,000 4 pounds. Plates issued under this Section shall expire 5 according to the multi-year procedure established by Section 6 3-414.1 of this Code. 7 (b) The design and color of the plates is wholly within 8 the discretion of the Secretary, except that the following 9 phrase shall be on the plates: "Mammograms Save Lives". The 10 Secretary may allow the plates to be issued as vanity plates 11 or personalized under Section 3-405.1 of the Code. The 12 Secretary shall prescribe stickers or decals as provided 13 under Section 3-412 of this Code. 14 (c) An applicant for the special plate shall be charged 15 a $25 fee for original issuance in addition to the 16 appropriate registration fee. Of this fee, $10 shall be 17 deposited into the Breast and Cervical Cancer Research Fund 18 and $15 shall be deposited into the Secretary of State 19 Special License Plate Fund, to be used by the Secretary to 20 help defray the administrative processing costs. 21 For each registration renewal period, a $25 fee, in 22 addition to the appropriate registration fee, shall be 23 charged. Of this fee, $23 shall be deposited into the Breast 24 and Cervical Cancer Research Fund and $2 shall be deposited 25 into the Secretary of State Special License Plate Fund. 26 (d) All money raised from the license plates and 27 deposited in the Breast and Cervical Cancer Research Fund 28 shall be paid, subject to appropriation by the General 29 Assembly, to the Illinois Department of Public Health for 30 grants to non-proft organizations and local health 31 departments for the provision and promotion of breast cancer 32 screening and diagnostic services.".
