90_HB3069ham001 LRB9009830MWmbam01 1 AMENDMENT TO HOUSE BILL 3069 2 AMENDMENT NO. . Amend House Bill 3069 by replacing 3 everything after the enacting clause with the following: 4 "Section 5. The Mental Health and Developmental 5 Disabilities Administrative Act is amended by adding Section 6 31b as follows: 7 (20 ILCS 1705/31b new) 8 Sec. 31b. Individuals waiting for community-based 9 services. The Department shall collect and maintain 10 information on the number of individuals with developmental 11 disabilities who have requested (either themselves or through 12 a parent or guardian) and are waiting for community-based 13 services that are funded through grants from the Department 14 to, and contracts with the Department and, local school 15 districts, individual service coordination organizations, and 16 community based organizations. The information shall 17 include, but is not limited to, the following: 18 (1) The number of those individuals waiting for 19 community-based residential services such as Community 20 Integrated Living Arrangements, supported apartments, 21 supported living, and other community living 22 arrangements. -2- LRB9009830MWmbam01 1 (2) The number of those individuals waiting for 2 services from intermediate care facilities for the 3 developmentally disabled. 4 (3) The number of those individuals waiting for 5 services from day and vocational programs such as day 6 activity programs, adult day care, developmental 7 training, sheltered workshops, supported employment, job 8 placement, and other day programs. 9 (4) The number of those individuals waiting for 10 family support services, respite care, personal 11 assistance services, case management services, early 12 intervention services, and other services not included in 13 paragraph (3). 14 (5) The ages of the those individuals waiting for 15 services under paragraphs (1), (2), (3), and (4). 16 (6) The ages of the parents of those individuals 17 waiting for services under paragraphs (1), (2), (3), and 18 (4). 19 (7) The Representative Districts in which those 20 individuals waiting for services live. 21 The information shall be updated annually. The information 22 collected shall be limited to information about individuals 23 who are eligible for services at the time of request or who 24 will be eligible to receive services within 12 months after 25 the date services are requested. The Department shall, on 26 request, make the information available to other agencies 27 serving persons with developmental disabilities.".
