1 AMENDMENT TO HOUSE BILL 1493
2 AMENDMENT NO. . Amend House Bill 1493 on page 6,
3 immediately below line 8, by inserting the following:
4 "Beginning on the effective date of this amendatory Act
5 of 1997 and until June 30, 2000, the fee for inspection and
6 testing shall be paid yearly at an annualized rate based on
7 the classifications and frequencies set forth in subsection
8 (f). The annualized fee for inspection and testing shall be
9 based on the rate of $55 per radiation machine for machines
10 located in dental offices and clinics and used solely for
11 dental diagnosis, located in veterinary offices and used
12 solely for diagnosis, or located in offices and clinics of
13 persons licensed under the Podiatric Medical Practice Act of
14 1987 and shall be based on the rate of $80 per radiation
15 machine for all other radiation machines. The Department may
16 adopt rules detailing the annualized rate structure."; and
17 on page 6, in line 9, by changing "The Department" to
18 "Beginning July 1, 2000, the Department"; and
19 on page 7, by replacing lines 11 through 15 with the
20 following:
21 "the Department. Beginning on the effective date of this
22 amendatory Act of 1997 and until June 30, 2000, the fee for
23 inspection review as described in this subsection shall be

1 paid yearly at an annualized rate based on the
2 classifications and frequencies set forth in subsection (f).
3 The annualized inspection review fee shall be based on the
4 rate of $25 per radiation machine. The Department may adopt
5 rules detailing the annualized rate structure. On and after
6 July 1, 2000, the Department shall by rule establish the
7 inspection review fee. The Department shall bill the operator
8 a $25 inspection review fee per radiation machine as soon as
9 practical after the Department receives the nondepartment
10 qualified inspector's radiation inspection report."; and
11 on page 9, in lines 15 and 16, by replacing "every 5 years"
12 with "every 5 years"; and
13 on page 9, in line 18, by inserting before "Fees" the
14 following:
15 "On and after January 1, 2000, the inspection and testing
16 frequency for radiation machines shall be established by rule
17 of the Department."; and
18 on page 9, in line 32, by replacing "every 2 years" with
19 "every 2 years"; and
20 on page 9, in line 34, by inserting before "Fees" the
21 following:
22 "On and after January 1, 2000, the inspection and testing
23 frequency for radiation machines shall be established by rule
24 of the Department."; and
25 on page 10, in line 17, by replacing "annually" with
26 "annually"; and
27 on page 10, in line 19, by inserting before "Fees" the
28 following:
29 "On and after January 1, 2000, the inspection and testing
30 frequency for radiation machines shall be established by rule
31 of the Department.".
