1 AMENDMENT TO HOUSE BILL 481
2 AMENDMENT NO. . Amend House Bill 481, on page 1 by
3 replacing lines 11 and 12 with the following:
4 "purpose other than that indicated by the sperm, ova or
5 embryo provider in a signed written consent form."; and
6 on page 1, by replacing lines 20 and 21 with the following:
7 "(d) Written consent is not required from donees of
8 human tissue, including sperm or ova, to a tissue bank
9 registered with the Department of".
