1     AN ACT concerning public aid.
 
2     Be it enacted by the People of the State of Illinois,
3 represented in the General Assembly:
 
4     Section 5. The Alzheimer's Special Care Disclosure Act is
5 amended by changing Sections 10 and 15 as follows:
 
6     (210 ILCS 4/10)
7     Sec. 10. Facility defined. As used in this Act, "facility"
8 means a facility licensed or permitted under the Nursing Home
9 Care Act, the Life Care Facility Act, the Assisted Living and
10 Shared Housing Act, the Community Living Facilities Licensing
11 Act, or subsection (a-20) of Section 30 of the Alternative
12 Health Care Delivery Act, or a facility designated as a
13 supportive living facility under Section 5-5.01a of the
14 Illinois Public Aid Code.

18     Sec. 15. Disclosure requirements. A facility that offers to
19 provide care for persons with Alzheimer's disease through an
20 Alzheimer's special care unit or center shall disclose to the a
21 State agency responsible for licensing or permitting the
22 facility and or to a potential or actual client of the facility

1 or such a client's representative the following information in
2 writing on request of the Agency or client:
3         (1) the form of care or treatment that distinguishes
4     the facility as suitable for persons with Alzheimer's
5     disease;
6         (2) the philosophy of the facility concerning the care
7     or treatment of persons with Alzheimer's disease;
8         (3) the facility's pre-admission, admission, and
9     discharge procedures;
10         (4) the facility's assessment, care planning, and
11     implementation guidelines in the care and treatment of
12     persons with Alzheimer's disease;
13         (5) the facility's minimum and maximum staffing
14     ratios, specifying the general licensed health care
15     provider to client ratio and the trainee health care
16     provider to client ratio;
17         (6) the facility's physical environment;
18         (7) activities available to clients at the facility;
19         (8) the role of family members in the care of clients
20     at the facility; and
21         (9) the costs of care and treatment under the program
22     or at the center.
