AMENDMENT TO HOUSE BILL 810

2     AMENDMENT NO. ______. Amend House Bill 810, AS AMENDED, by
3 replacing everything after the enacting clause with the
4 following:
 
5     "Section 5. The Alzheimer's Special Care Disclosure Act is
6 amended by changing Sections 10 and 15 as follows:
 
7     (210 ILCS 4/10)
8     Sec. 10. Facility defined. As used in this Act, "facility"
9 means a facility licensed or permitted under the Nursing Home
10 Care Act, the Life Care Facility Act, the Assisted Living and
11 Shared Housing Act, the Community Living Facilities Licensing
12 Act, or subsection (a-20) of Section 30 of the Alternative
13 Health Care Delivery Act, or a facility designated as a
14 supportive living facility under Section 5-5.01a of the
15 Illinois Public Aid Code.

3     Sec. 15. Disclosure requirements. A facility that offers to
4 provide care for persons with Alzheimer's disease through an
5 Alzheimer's special care unit or center shall disclose to the a
6 State agency responsible for licensing or permitting the
7 facility and or to a potential or actual client of the facility
8 or such a client's representative the following information in
9 writing on request of the Agency or client:
10         (1) the form of care or treatment that distinguishes
11     the facility as suitable for persons with Alzheimer's
12     disease;
13         (2) the philosophy of the facility concerning the care
14     or treatment of persons with Alzheimer's disease;
15         (3) the facility's pre-admission, admission, and
16     discharge procedures;
17         (4) the facility's assessment, care planning, and
18     implementation guidelines in the care and treatment of
19     persons with Alzheimer's disease;
20         (5) the facility's minimum and maximum staffing
21     ratios, specifying the general licensed health care
22     provider to client ratio and the trainee health care
23     provider to client ratio;
24         (6) the facility's physical environment;
25         (7) activities available to clients at the facility;

1         (8) the role of family members in the care of clients
2     at the facility; and
3         (9) the costs of care and treatment under the program
4     or at the center.
