AMENDMENT TO HOUSE BILL 4056

AMENDMENT NO. ______. Amend House Bill 4056, AS AMENDED, with reference to page and line numbers of Senate Amendment No. 1 as follows:

on page 8, line 16, by deleting "Illinois licensed"; and

on page 10, by replacing lines 3 through 15 with the following:

"(3) The transfer fee and any additional fees or charges.

(4) If the dog or cat was returned by an adopter, then the date and reason for the return.

(5) The following written statement: "A copy of our policy regarding warranties, refunds, or returns is available upon request.".

(6) If the dog or cat originated at an Illinois licensed animal shelter or animal control facility, the license number of the animal shelter or animal control facility issued by the Illinois Department of Agriculture.

(7) If the dog or cat originated at an out-of-state animal shelter or animal control facility, a copy of the certificate of veterinary inspection."

on page 13, by deleting lines 14 through 20; and

on page 13, line 21, by replacing "(b) (a)" with "(a)"; and

on page 14, lines 7 and 8, by deleting "Illinois licensed"; and

on page 14, lines 8 and 9, by deleting "or dog dealer".
