AMENDMENT TO HOUSE BILL 6065

AMENDMENT NO. ______. Amend House Bill 6065, AS AMENDED, with reference to page and line numbers of House Amendment No. 1, on page 3, by replacing lines 14 through 18 with the following:

""Health care provider" means a physician licensed to practice medicine in all of its branches, advanced practice nurse who has a written collaborative agreement with a collaborating physician who authorizes provision of diabetes care, or a physician assistant who has a written supervision agreement with a supervising physician who authorizes provision of diabetes care."; and

on page 4, by replacing lines 5 through 16 with the following:

"(a) A diabetes care plan must be signed and submitted by a parent or guardian for any student with diabetes who seeks assistance with diabetes care in the school setting. The diabetes care plan shall include the treating health care provider's instructions concerning the student's diabetes management during the school day, including a copy of a prescription and the methods of insulin administration.".
