3 line 22, by deleting "person"; and

4 on page 11, line 4, by deleting "or"; and

5 on page 11, line 5, by replacing "provider." with "provider;
6 or"; and

7 on page 11, immediately below line 5, by inserting the
8 following:
9 "(6) a person.".
