Short description: 
CD CORR-RELEASED PRISONERS-MEN                                             

Synopsis of Bill as introduced:
        Amends  the  Unified  Code  of  Corrections.  Provides  for   the      
   identification  of  persons  committed  to  Department  of Corrections      
   facilities who:  (1)  are  reasonably  believed  to  be  dangerous  to      
   themselves  or others; and (2) have a mental disorder. Provides that a      
   team consisting of representatives of the Department  of  Corrections,      
   the    Department    of    Human   Services   specifically   including      
   representatives knowledgeable  in  the  treatment  of  alcoholism  and      
   substance   abuse   and   those  knowledgeable  in  the  treatment  of      
   developmental disabilities, the appropriate  community  mental  health      
   facility,  and  other mental health service providers, as appropriate,      
   shall develop a plan for each such  identified  committed  person,  as      
   determined  necessary  by  the  team,  for  delivery  of treatment and      
   support services to the committed person upon release  of  the  person      
   from the correctional facility.
