1 HOUSE RESOLUTION
2 WHEREAS, The members of the Illinois House of
3 Representatives wish to honor the recipients of awards given
4 at the 1999 Congress of Delegates Awards Dinner, held July
5 17, 1999, and presented by the Illinois Academy of Family
6 Physicians Foundation; and
7 WHEREAS, The awards were presented by Steven M. Wilk,
8 M.D., President of the IAFP; and
9 WHEREAS, Distinguished Service Awards, received by Board
10 members completing three year terms in 1999, were presented
11 to: Michael Brummer, M.D., Elizabeth Burns, M.D., and
12 1998-1999 Chair of the Board Susan Rife, D.O.; distinguished
13 service awards were presented to members of the Executive
14 committee: Immediate Past President and AAFP Alternate
15 Delegate Kelly Carroll, M.D., Speaker Ronald L. Johnson,
16 M.D., Treasurer Robert Sawicki, M.D., Vice Speaker Tim J.
17 Vega, M.D., Resident Board Member Matthew T. Johnson, M.D.,
18 Student Board Member Daniel W. Kattenbraker, and Student
19 Board Member Deepak Khuntia; and
20 WHEREAS, Receiving the Foundation Award, presented by
21 Foundation President John Adams, M.D., were: First Resident
22 Board Member Mohammad A. Malik, M.D., and First Student
23 Member Kerri Harting; and
24 WHEREAS, President-elect Christine Petty, M.D. presented
25 the President's Medallion to Steven M. Wilk, M.D.; Steven M.
26 Wilk presented the President's Award to IAFP deputy executive
27 vice president Sally Winkelman and president of the IAFP
28 student chapter Kerri Harting; and
29 WHEREAS, Nominees for Family Physician Teacher of the
30 Year were: Jacqueline Bardwell, M.D., Stephen Bennett, M.D.,
31 Rober Ewart, M.D., Jerry Goddard, M.D., Christine Jacobs,

1 M.D., Donna Lawlor, M.D., Richard Lord, M.D., and Donna
2 White, M.D.; the winner of the 1999 Family Physician Teacher
3 of the Year was Michael Ahearn, M.D.; and
4 WHEREAS, Nominees for the Family Physician of the Year
5 were: Christopher K. Kubik, M.D., Gregory T. Winters, M.D.,
6 David L. Reese, M.D., Meehro M. Patel, M.D., and Jack P.
7 Spaniol, M.D.; the winner of the 1999 Family Physician of the
8 Year was Secundino E. Rubio, M.D.; therefore, be it
9 RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE
10 NINETY-FIRST GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that
11 we congratulate the award recipients from the 1999 Congress
12 of Delegates Awards Dinner; because of the dedication and
13 hard work of these men and women, the awards received were
14 truly deserved; and be it further
15 RESOLVED, That a suitable copy of this resolution be
16 given to all the award recipients.
