1 AMENDMENT TO SENATE BILL 83
2 AMENDMENT NO. . Amend Senate Bill 83 on page 1,
3 immediately below line 3, by inserting the following:
4 "Section 3. The Department of Human Services Act is
5 amended by adding Section 10-12 as follows:
6 (20 ILCS 1305/10-12 new)
7 Sec. 10-12. Grants to the American Diabetes Association.
8 The Department of Human Services shall make grants to the
9 American Diabetes Association for research on diabetes from
10 appropriations to the Department from the American Diabetes
11 Fund, a special fund created in the State Treasury."; and
12 on page 1, line 25, by changing "5.449 and 5.450" to "5.449,
13 5.450, and 5.451"; and
14 on page 2, below line 2, by inserting the following:
15 "(30 ILCS 105/5.451 new)
16 Sec. 5.451. The American Diabetes Association Fund.";
17 and
18 on page 2, lines 4 and 5, by changing "Sections 507R and
19 507S" to "Sections 507R, 507S, and 507T"; and

1 on page 2, below line 29, by inserting the following:
2 "(35 ILCS 5/507T new)
3 Sec. 507T. The American Diabetes Association check off.
4 The Department shall print on its standard individual income
5 tax form a provision indicating that if the taxpayer wishes
6 to contribute to the American Diabetes Association Fund, as
7 authorized by this amendatory Act of 1997, he or she may do
8 so by stating the amount of the contribution (not less than
9 $1) on the return and that the contribution will reduce the
10 taxpayer's refund or increase the amount of payment to
11 accompany the return. Failure to remit any amount of
12 increased payment shall reduce the contribution accordingly.
13 This Section shall not apply to any amended return."; and
14 on page 3, line 24, by changing "Cancer Fund, to" to "Cancer
15 Fund, to the American Diabetes Association Fund, to"; and
16 on page 4, line 24, by changing "Fund," to "Fund, the
17 American Diabetes Association Fund,".
