Amend House Bill 2645, AS AMENDED, 3 in Section 25 of the bill, in Sec. 356g, in subsection (c), 4 in the second sentence, by replacing "patient to be 5 appropriate" with "patient in accordance with utilization 6 review guidelines which are consistent with clinical practice 7 guidelines developed by the appropriate medical specialty 8 society to be appropriate"; and 9 in Section 25 of the bill, in Sec. 356w, in subsection (a), 10 by inserting immediately below the last line of subsection 11 (a) the following: 12 "Coverage under this Section shall include reconstructive 13 surgery that is performed on abnormal structures of the body 14 caused by congenital defects, developmental abnormalities, 15 trauma, infection, tumors, or disease. Reconstructive surgery 16 is generally performed to improve function, but may also be 17 done to approximate a normal appearance. Coverage shall not 18 include cosmetic surgery. "Cosmetic surgery" for purposes of 19 this Section and as defined by the American Medical 20 Association means surgery performed to reshape normal 21 structures of the body to improve the patient's appearance 22 and self-esteem."; and 23 in Section 30 of the bill, in Sec. 4-6.1, in subsection (b), -2- LRB9008967JSgcam04 1 in the second sentence, by replacing "patient to be 2 appropriate" with "patient in accordance with utilization 3 review guidelines which are consistent with clinical practice 4 guidelines developed by the appropriate medical specialty 5 society to be appropriate".
