Updating the database of the Illinois Compiled Statutes (ILCS) is an ongoing process. Because the statute database is maintained primarily for legislative drafting purposes, statutory changes are sometimes included in the statute database before they take effect. If the source note at the end of a Section of the statutes includes a Public Act that has not yet taken effect, the version of the law that is currently in effect may have already been removed from the database and you should refer to that Public Act to see the changes made to the current law.

    Sec. 15. Registration required.
    (a) A certificate of registration issued by the Department shall be required prior to the operation of any establishment or multi-type establishment. The operator of the facility shall file an application for a certificate of registration with the Department that shall be accompanied by the requisite fee, as determined by the Department, and include all of the following information:
        (1) The applicant's (operator) name, address,
    
telephone number, and age. In order to qualify for a certificate of registration under this Act, an applicant must be at least 18 years of age.
        (2) The name, address, and phone number of the
    
establishment.
        (3) The type and year of manufacture of the
    
equipment proposed to be used for tattooing or body piercing.
        (4) The sterilization and operation procedures to
    
be used by the establishment.
        (5) Any other information required by the
    
Department.
    (b) If the operator owns or operates more than one establishment, the operator shall file a separate application for each facility owned or operated.
