1 AMENDMENT TO SENATE BILL 3414
2     AMENDMENT NO. ______. Amend Senate Bill 3414 on page 1,
3 line 20, by deleting "the policy is in full alignment with
4 Medicare and"; and

5 on page 5, line 7, after the semicolon, by inserting "the
6 ordering provider is not required to obtain continuing medical
7 education in order to prescribe a continuous glucose
8 monitor;"; and

9 on page 6, by replacing lines 3 through 6 with the following:
10         "(7) prior authorization is required for a
11     prescription of a continuous glucose monitor; once a
12     continuous glucose monitor is prescribed, the prior
13     authorization shall be approved for a 12-month period.".
