1 AMENDMENT TO SENATE BILL 3137
2 AMENDMENT NO. ______. Amend Senate Bill 3137 on page 1, by
3 replacing lines 11 and 12 with the following:
4 "representative, if known, of the patient as specified in this
5 Section. The facility shall attempt to provide verbal notice
6 to the personal representative, if known, of the patient"; and

7 on page 2, by replacing lines 8 and 9 with the following:
8 "representative, if known, of the recipient of services as
9 specified in this Section. The facility shall attempt to
10 provide verbal notice to the personal representative, if"; and

11 on page 2, line 12, by inserting ", if known," after
12 "representative".
