4 on page 7, by replacing line 2 with "following information, if

6 on page 7, lines 6 and 7, by deleting ", if known or knowable";
7 and

8 on page 7, line 9, by deleting ", if known or knowable"; and

9 on page 7, by replacing lines 13 and 14 with "the deceased's
10 body was found in a prompt manner"; and

11 on page 7, line 15, by deleting "determined".
