1 AMENDMENT TO SENATE BILL 336
2     AMENDMENT NO. ______. Amend Senate Bill 336 by replacing
3 everything after the enacting clause with the following:

4     "Section 5. The Vital Records Act is amended by adding
5 Section 17.1 as follows:

6     (410 ILCS 535/17.1 new)
7     Sec. 17.1. Redacted certificate of birth.
8     (a) The Department shall issue a certificate of birth with
9 the identity of the certifier redacted upon request by:
10         (1) any person named on the certificate of birth, if
11     the person is 18 years of age or older;
12         (2) a parent of the person named on the certificate of
13     birth;
14         (3) the legal representative of the person named on
15     the certificate of birth; or
16         (4) an attorney at law authorized in writing by the

1         person named on the certificate of birth.
2     (b) The Department may adopt any rules necessary to
3 implement the provisions of this Section.

4     Section 99. Effective date. This Act takes effect 120 days
5 after becoming law.".
