2 AMENDMENT NO. ______. Amend Senate Bill 455, AS AMENDED,
3 with reference to page and line numbers of Senate Amendment No.
4 1, as follows:

5 on page 2, line 6, after "if", by inserting "all of the
6 following conditions in this subsection (b) are met"; and

7 on page 3, lines 4 and 5, by replacing "to the prescriber" with
8 "in accordance with this subsection (c)".
