AMENDMENT TO HOUSE BILL 5079

    AMENDMENT NO. ______. Amend House Bill 5079 on page 2, by

replacing line 21 through line 23 with the following:

    "care provider, who ordered or provided the services in 
5
    question and who is licensed under the
Medical Practice Act 
6
    of 1987 is a physician licensed  to practice medicine in all 
7
