AMENDMENT TO HOUSE BILL 2325

AMENDMENT NO. ______. Amend House Bill 2325, AS AMENDED, with reference to page and line numbers of Senate Amendment No. 1 as follows:

on page 3, by replacing line 7 with the following:

"4. Within 10 days after the employee's or member's Upon termination or reduction in hours below the"; and

Until January 1, 2010, in the event the employee or member contacts the insurer regarding continuation rights and advises that notice has not been provided by the employer or group policyholder, the insurer shall provide a written explanation to the employee or member of the employee's or member's continuation rights pursuant to this Section."; and

on page 5, line 13, after "presented", by inserting "or mailed"; and

on page 6, line 7, after "presented", by inserting "or mailed"; and

on page 6, line 15, by replacing "sent" with "presented or mailed"; and

on page 12, by replacing line 1 with the following:
