1 AMENDMENT TO SENATE BILL 654
2     AMENDMENT NO. ______. Amend Senate Bill 654 on page 3,
3 immediately below line 6, by inserting the following:

4     "Section 15. The Illinois State Diabetes Commission Act is
5 amended by changing Sections 5 and 10 as follows:

6     (20 ILCS 4055/5)
7     (Section scheduled to be repealed on January 1, 2010)
8     Sec. 5. Illinois State Diabetes Commission. There is hereby
9 established within the Department of Human Services the
10 Illinois State Diabetes Commission. The Commission shall
11 consist of members that are residents of this State and shall
12 include an Executive Committee appointed by the Secretary of
13 Human Services ~~as provided by rule~~. The members of the
14 Commission shall be appointed by the Secretary of Human
15 Services as follows:
16         (1) The Secretary of Human Services or the Secretary's

1     designee, who shall serve as chairperson of the Commission.
2         (2) Physicians who are board certified in
3     endocrinology, with at least one physician with expertise
4     and experience in the treatment of childhood diabetes and
5     at least one physician with expertise and experience in the
6     treatment of adult onset diabetes.
7         (3) Health care professionals with expertise and
8     experience in the prevention, treatment, and control of
9     diabetes.
10         (4) Representatives of organizations or groups that
11     advocate on behalf of persons suffering from diabetes.
12         (5) Representatives of voluntary health organizations
13     or advocacy groups with an interest in the prevention,
14     treatment, and control of diabetes.
15         (6) Members of the public who have been diagnosed with
16     diabetes.
17     The Secretary of Human Services may appoint additional
18 members deemed necessary and appropriate by the Secretary.

22     Sec. 10. Members; meetings. Members of the Commission shall
23 be appointed within one year after the effective date of this
24 amendatory Act of the 95th General Assembly~~by December 1,~~
25 ~~2006~~. A member shall continue to serve until his or her

1 successor is duly appointed and qualified.
2     Meetings shall be held 3 times per year or at the call of
3 the Commission chairperson.
