AMENDMENT TO HOUSE BILL 4249

2     AMENDMENT NO. ______. Amend House Bill 4249, AS AMENDED,
3 with reference to page and line numbers of Senate Amendment No.
4 2, as follows:
 
5 on page 13, line 13, after "expire", by inserting "for a period
6 of less than 5 years"; and
 
7 on page 13, immediately below line 20, by inserting the
8 following:
9     "(b-5) A person seeking restoration of a license that has
10 been expired or placed on inactive status for a period of 5
11 years or more may have the license restored by making
12 application to the Department and filing proof acceptable to
13 the Department of fitness to have the license restored. Proof
14 may include (i) sworn evidence of active practice in another
15 jurisdiction, (ii) an affidavit attesting to military service
16 of the type set forth in subsection (c) of this Section, (iii)

1 proof of passage of the Environmental Health Proficiency
2 Examination during the period in which the license lapsed or
3 was placed on inactive status, or (iv) sworn evidence
4 acceptable to the Department of lawful practice under the
5 supervision of an environmental health practitioner licensed
6 under this Act. Except as otherwise stated in this Section, an
7 applicant for restoration under this Section must pay any
8 restoration fees required under this Act and provide proof of
9 meeting continuing education requirements during the 2-year
10 period immediately prior to restoration."; and
 
11 on page 15, line 13, by deleting "impairment".
