| 2 |
| Be it enacted by the People of the State of Illinois,
| 3 |
| represented in the General Assembly:
| 4 |

| Sections 4-8.03, 5-7.03, and 6-35.03 as follows:
| 6 |
| (10 ILCS 5/4-8.03) (from Ch. 46, par. 4-8.03)
| 7 |
| Sec. 4-8.03. The State Board of Elections shall design a | 8 |
| registration
record card which, except as otherwise provided in | 9 |

be used in triplicate by all election | 10 |
| authorities
in the State, except those election authorities | 11 |
| adopting a computer-based
voter registration file authorized | 12 |
| under Section 4-33. The Board shall prescribe the form and | 13 |
| specifications, including
but not limited to the weight of | 14 |
| paper, color and print of such cards.
Such cards shall contain | 15 |
| boxes or spaces for the information required under
Sections 4-8 | 16 |
| and 4-21 of this Code; provided, that such cards shall also
| 17 |
| contain a box or space for the applicant's social security | 18 |
| number, which
