HOUSE RESOLUTION

 
2    WHEREAS, Dr. Daniel Hoffman, the son of a postman, was born
3in Henry, Illinois; he taught himself to read by memorizing
4children's books; one of his favorite books, Dr. Dan the
5Bandage Man, instilled a lifelong love for medicine at a young
6age; and
 
7    WHEREAS, Dr. Hoffman graduated from the University of
8Illinois at Urbana in 1974; he knew the medical profession was
9the only future for him, so he attended medical school at the
10University of Illinois College of Medicine in Peoria; and
 
11    WHEREAS, During his time at medical school, Dr. Hoffman met
12his future wife, Linda Sargent; their marriage in 1977 was the
13beginning of a personal and professional partnership that
14produced two sons, Doug and Eric, and a successful medical
15practice; and
 
16    WHEREAS, From 1977 until 1981, Dr. Hoffman was a resident
17in internal medicine at St. Francis Medical Center; he wanted
18to practice medicine and raise his family in a town small
19enough where he could get to know everyone, but still big
20enough to have a McDonald's; with encouragement from his close
21friend, Dr. Charles Longwell, he left St. Francis Medical
22Center and moved to Good Samaritan Hospital in Mt. Vernon;

1using his wife's training and experience as a certified medical
2office assistant and transcriptionist, they worked hard to
3establish his new practice, specializing in internal medicine
4and geriatrics; within three weeks, his schedule was full, and
5it remained that way for 37 years; and
 
6    WHEREAS, In 2007, Dr. Hoffman became a Certified Physician
7Executive at the American College of Physician Executives; in
82008, he acquired his master's in medical management from Heinz
9College at Carnegie Mellon University; and
 
10    WHEREAS, Dr. Hoffman and his wife contribute to many
11philanthropic organizations, including the Cedarhurst Center
12for the Arts, where Linda became a Counselor Emeritus, and they
13have both donated their time, service, and financial support;
14both have a keen interest in promoting education through
15various venues, and Dr. Hoffman served on the board of Mt.
16Vernon Township High School for seven years, including serving
17as president; for the past 18 years, he has been the SSM Good
18Samaritan Hospital's Administrative Medical Director; he also
19puts his woodworking skills to the test each year for the YMCA
20Derby Days fundraiser by donating the table centerpieces as
21well as the wooden horses used in the fundraiser; and
 
22    WHEREAS, Dr. Hoffman and Linda plan to spend their
23retirement cooking, cruising, playing golf, and gardening;

1most importantly, they are eager to spend more time with their
2children and grandchildren, including Andrew, Caleb, and a baby
3on the way; therefore, be it
 
4    RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE ONE
5HUNDREDTH GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that we
6congratulate Dr. Daniel Hoffman on the occasion of his
7retirement from practicing medicine for 37 years; we commend
8his hard work, dedication, and accomplishments and thank him
9for his contributions to his community and the field he has
10loved from a young age; and be it further
 
11    RESOLVED, That a suitable copy of this resolution be
12presented to Dr. Hoffman as an expression of our esteem and
13respect.
