1    AN ACT concerning State government.
 
2    Be it enacted by the People of the State of Illinois,
3represented in the General Assembly:
 
4    Section 5. The Mental Health and Developmental
5Disabilities Administrative Act is amended by changing Section
653 as follows:
 
7    (20 ILCS 1705/53)  (from Ch. 91 1/2, par. 100-53)
8    Sec. 53. The Department shall create a consistent case
9coordination system for persons with a developmental
10disability who receive services provided or funded by the
11Department. The objectives of this system shall be to ensure
12that a full range of an individual's needs is identified and
13assessed through statewide use of an individual client
14assessment tool; to ensure that each individual actually
15receives, in the most effective and efficient combination and
16sequence, the full range and continuum of services needed; to
17ensure that all available resources are applied appropriately
18to each individual served; and to provide a systematic
19procedure for serving individuals which generates among and
20within the local service delivery agencies information
21required for effective system management.
22    Each individual residing in a community integrated living
23arrangement shall receive an annual assessment to screen that

1individual for any health issues or risks. Beginning July 1,
22019, each individual shall receive his or her annual client
3assessment via a web-based, electronic screening tool. The
4electronic screening tool shall replace the current
5paper-based assessment. A provider may make a request, along
6with justifications, to the Department to complete the
7assessment on paper. Subject to appropriation, the Department
8may contract with a third-party entity to create and implement
9the web-based, electronic screening tool. The Department shall
10make changes to its rules in the Illinois Administrative Code
11to incorporate a web-based, electronic assessment tool.
12    "Case coordination" means a mechanism for linking and
13coordinating segments of the service delivery system to ensure
14the most comprehensive program for meeting an individual
15client's needs. It facilitates client movement through an array
16of services so that at any given time, services received are
17matched to the needs of the individual.
