HOUSE RESOLUTION

WHEREAS, Several State-operated facilities and State psychiatric hospitals that serve adults with developmental disabilities and mental illness have closed in recent years; and

WHEREAS, It is critical that former residents of these facilities do not lose vital health care services as they transition to a new service provider, whether it be a community-based agency, another State-operated facility or State psychiatric hospital, or home care; therefore, be it

RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE NINETY-SIXTH GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that the Department of Human Services shall file a report detailing the number of individuals that have been transferred or transitioned out of a State-operated facility or State psychiatric hospital due to facility closures; where these individuals were transferred or transitioned to; how these individuals are meeting their health care needs following transfer or transition; whether these individuals have experienced a break in health care service and, if applicable, the length of time these individuals went without health care service; and how these individuals are meeting their health care needs one year later; and be it further

RESOLVED, That the Department shall submit this report to the Governor and the General Assembly by January 31, 2011 and January 31 of every year thereafter, and that the report shall be made available to the public; and be it further

RESOLVED, That a suitable copy of this resolution be delivered to the Secretary of Human Services.
