HOUSE RESOLUTION

 
2     WHEREAS, Several State-operated developmental disability
3 or mental health facilities that serve adults with
4 developmental disabilities and mental illness have closed in
5 recent years; and
 
6     WHEREAS, It is critical that former residents of these
7 facilities receive services and supports necessary to meet
8 their needs, whether they transition to a community-based
9 agency, another State-operated facility, or home care;
10 therefore, be it
 
11     RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE
12 NINETY-SIXTH GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that
13 when the Department of Human Services closes a State-operated
14 developmental disability or mental health facility after the
15 adoption of this resolution, the Department shall track and
16 report how these individuals' health care and other service and
17 support needs are being met for one year after the closure of
18 the State-operated developmental disability or mental health
19 facility, detailing the number of individuals that have been
20 transferred or transitioned out of a State-operated
21 developmental disability or mental health facility due to
22 facility closures; the setting to which these individuals were
23 transferred or transitioned to; and how these individuals'

1 health care and other service and support needs are being met
2 following transfer or transition from a State-operated
3 developmental disability or mental health facility; and be it
4 further
 
5     RESOLVED, That the Department shall submit this report to
6 the Governor and the General Assembly by January 31st of each
7 year following the closure of a facility, and that the report
8 shall be made available to the public; and be it further
 
9     RESOLVED, That a suitable copy of this resolution be
10 delivered to the Secretary of Human Services.
