HOUSE RESOLUTION

WHEREAS, Several State-operated developmental disability or mental health facilities that serve adults with developmental disabilities and mental illness have closed in recent years; and

WHEREAS, It is critical that former residents of these facilities receive services and supports necessary to meet their needs, whether they transition to a community-based agency, another State-operated facility, or home care; therefore, be it

RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE NINETY-SIXTH GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that when the Department of Human Services closes a State-operated developmental disability or mental health facility after the adoption of this resolution, the Department shall track and report how these individuals' health care and other service and support needs are being met for one year after the closure of the State-operated developmental disability or mental health facility, detailing the number of individuals that have been transferred or transitioned out of a State-operated developmental disability or mental health facility due to facility closures; the setting to which these individuals were transferred or transitioned to; and how these individuals' health care and other service and support needs are being met following transfer or transition from a State-operated developmental disability or mental health facility; and be it further

RESOLVED, That the Department shall submit this report to the Governor and the General Assembly by January 31st of each year following the closure of a facility, and that the report shall be made available to the public; and be it further

RESOLVED, That a suitable copy of this resolution be delivered to the Secretary of Human Services.
