1 HOUSE RESOLUTION

2     WHEREAS, The members of the Illinois House of
3 Representatives are pleased to congratulate Sally Stachniak on
4 the occasion of her retirement from the McHenry County Mental
5 Health Board; and

6     WHEREAS, Sally Stachniak has devoted over 20 years of
7 dedicated service to the citizens of McHenry County; her most
8 current position was that of Associate Director of Children's
9 Services and Planning of the McHenry County Mental Health
10 Board; and

11     WHEREAS, Sally Stachniak was instrumental in authoring the
12 Family CARE grant and in starting the Wraparound, Project
13 Success, and Adoption Support programs; she also assisted many
14 agencies in securing additional funding through grants,
15 especially Family Services and the Family Health Partnership;
16 and

17     WHEREAS, Sally Stachniak's dedication to her profession
18 and her willingness to serve her community make her a model for
19 the people of the State of Illinois; therefore, be it

20     RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE
21 NINETY-FIFTH GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that we

1 congratulate Sally Stachniak on the occasion of her retirement
2 from the McHenry County Mental Health Board and wish her
3 continued success and happiness in the future; and be it
4 further

5     RESOLVED, That a suitable copy of this resolution be
6 presented to Sally Stachniak as a symbol of our esteem and
7 respect.
