1    AN ACT concerning health.
 
2    Be it enacted by the People of the State of Illinois,
3represented in the General Assembly:
 
4    Section 5. The Mental Health and Developmental
5Disabilities Code is amended by adding Section 3-911 as
6follows:
 
7    (405 ILCS 5/3-911 new)
8    Sec. 3-911. Aftercare plan.
9    (a) The Department of Human Services when a recipient of
10services under this Code, whether admitted on a voluntary or
11involuntary basis, is being discharged from an inpatient
12facility, shall provide the recipient and the recipient's
13conservator, guardian, or other legally authorized
14representative a written aftercare plan prior to the
15recipient's discharge from the facility. The written aftercare
16plan shall include, to the extent known, all of the following
17components:
18        (1) the nature of the illness and followup required;
19        (2) medications including side effects and dosage
20    schedules;
21        (3) if the recipient was given an informed consent
22    form with his or her medications, the form shall satisfy
23    the requirement for information on side effects of the

1    medications;
2        (4) expected course of recovery;
3        (5) recommendations regarding treatment that is
4    relevant to the recipient's care;
5        (6) referrals to providers of medical and mental
6    health services; and
7        (7) other relevant information.
8    (b) The recipient shall be advised by facility personnel
9that he or she may designate another person to receive a copy
10of the aftercare plan.
