SENATE RESOLUTION

 
2    WHEREAS, The members of the Illinois Senate are saddened to
3learn of the death of Dr. Thomas E. "Tom" Theobald OD of
4Jacksonville, Florida, formerly of Bloomington, who passed
5away on February 2, 2019; and
 
6    WHEREAS, Tom Theobald was born in Springfield to Paul and
7Marguerite (Phillips) Theobald on October 15, 1940; he
8graduated from Tri-City High School; he then earned his
9Bachelor of Science from Culver-Stockton College, where he was
10also a member of Sigma Phi Epsilon; he earned his Doctor of
11Optometry degree from Southern College of Optometry, where he
12was a member of Omega Delta; he married Joyce McMaster in
13Memphis, Tennessee on July 6, 1963; she preceded him in death;
14he later married Lynn Morford in Buffalo Hart on March 22,
152003; and
 
16    WHEREAS, Tom Theobald was a Captain in the Army Medical
17Service Corps and served as an optometry officer at Valley
18Forge General Hospital in Pennsylvania during the Vietnam era;
19he had an optometry practice in Normal and worked as an adjunct
20professor at Illinois State University; and
 
21    WHEREAS, Tom Theobald was active in many civic
22organizations; he served as Chairman Emeritus of Citizens

1Savings Bank (now Busey Bank) and as Vice Chairman of the
2Bloomington-Normal Sanitary District; he was a fellow of the
3American Academy of Optometry and held several offices in local
4and state optometric associations and the American Optometric
5Association; as President of the Normal Chamber of Commerce, he
6worked to create the McLean County Association of Commerce and
7Industry; he was a former member of the Normal Rotary Club and
8a former board member of the Bloomington Country Club; and
 
9    WHEREAS, Tom Theobald enjoyed creating computer
10applications with his friend, Dr. Gerry McKean; he earned his
11commercial pilot's license and enjoyed golfing, scuba diving,
12traveling, watchmaking, and, most of all, his family; and
 
13    WHEREAS, Tom Theobald was preceded in death by his sister,
14Linda (Paul) Smith; and
 
15    WHEREAS, Tom Theobald is survived by his wife, Lynn; his
16daughters, Ann (Brent) Hollenbeck and Jane (Marc) Stein; his
17brother, Dale (Linda) Theobald; his grandchildren, Kate
18Hollenbeck, Olivia Hollenbeck, Ellee Stein, and Nathan Stein;
19his sister-in-law, Jane (Clay) Wilson; his brother-in-law,
20Scott (Susan) Morford; and several cousins, nieces, and
21nephews; therefore, be it
 
22    RESOLVED, BY THE SENATE OF THE ONE HUNDRED FIRST GENERAL

1ASSEMBLY OF THE STATE OF ILLINOIS, that we mourn the passing of
2Dr. Thomas E. "Tom" Theobald OD and extend our sincere
3condolences to his family, friends, and all who knew and loved
4him; and be it further
 
5    RESOLVED, That a suitable copy of this resolution be
6presented to the family of Tom Theobald as an expression of our
7deepest sympathy.
