SENATE RESOLUTION

 
2     WHEREAS, The members of the Illinois Senate wish to express
3 their condolences to the family and friends of David M. Hayes
4 of Roseville, who passed away on August 23, 2007; and
 
5     WHEREAS, He was born on October 6, 1961 in Monmouth, the
6 son of Gary and Donna Smith Hayes; he attended the Roseville
7 schools; he married Karen Grant on July 13, 1985 in Monmouth;
8 and
 
9     WHEREAS, David Hayes worked with the Swan Creek Volunteer
10 Fire Department from 1979 to 1985 and the Avon Volunteer Fire
11 Department from 1985 to 1987; he worked as an Emergency Medical
12 Technician for United Ambulance from 1980 to 1994 and for the
13 Roseville Area Ambulance Service from 1979 until his passing;
14 he worked for the Corman Memorial Homes; and he was a paramedic
15 for Galesburg Hospital Ambulance Service from 1987 to 2006; and
 
16     WHEREAS, He was a member and past president of Roseville
17 Ambulance Service, a member of the Roseville Fire Department,
18 and District Fire Marshal for the R.S.P. & E. Fire Protection
19 District from 1995 to 2006; and he was Assistant Chief of the
20 Roseville Rescue Squad; and
 
21     WHEREAS, David Hayes had over 27 years in the Fire Service

1 and the Emergency Medical Field and over 15 years as an
2 Emergency Medical Technician Instructor; he was a CPR
3 Instructor for the American Red Cross; he assisted in
4 organizing the Roseville Fire Department Rescue Squad in 1991;
5 and he had many hours of continuing education and volunteer
6 community service in both the EMS and Fire Service fields; and
 
7     WHEREAS, David Hayes was a member and past Master of the
8 Roseville Masonic Lodge #519 and a member of the Scottish Rite
9 Bodies, Valley of Peoria; he was a Shriner for Mohammed Shrine
10 of Peoria, a member and president of the Roseville Township
11 Cemetery, and a member of the Spark Plug Collectors; in
12 addition, he was a member of the Swan Creek United Methodist
13 Church and the Warren County 911 board; and
 
14     WHEREAS, He was preceded in death by his maternal
15 grandparents and his paternal grandfather; and
 
16     WHEREAS, The passing of David Hayes is deeply felt by his
17 wife, Karen; his parents, Gary and Donna Hayes; his brother,
18 Jeff Hayes; and his paternal grandmother, Blanche Hayes;
19 therefore, be it
 
20     RESOLVED, BY THE SENATE OF THE NINETY-FIFTH GENERAL
21 ASSEMBLY OF THE STATE OF ILLINOIS, that we mourn, along with
22 his family and friends, the passing of this man dedicated to

1 public service and saving lives, David M. Hayes; and be it
2 further
 
3     RESOLVED, That a suitable copy of this resolution be
4 presented to the family of David M. Hayes as a symbol of our
5 sincere sympathy.
