AMENDMENT TO SENATE BILL 810

2     AMENDMENT NO. ______. Amend Senate Bill 810 by replacing
3 everything after the enacting clause with the following:
 
4     "Section 5. The State Employees Group Insurance Act of 1971
5 is amended by adding Section 6.16 as follows:
 
6     (5 ILCS 375/6.16 new)
7     Sec. 6.16. Diabetes collaborative prevention and wellness
8 pilot program. The Department shall develop and implement a
9 3-year pilot program of collaborative prevention and wellness
10 services for State employees with diabetes residing in Cook
11 County or Champaign County to improve their health outcomes and
12 to manage and reduce medical care expenditures related to those
13 individuals.
14     The program shall facilitate the coordination of
15 physicians and pharmacists in their care of program
16 participants, including but not limited to reimbursement of

1 pharmacists for face-to-face collaborative prevention and
2 wellness services. The Department may offer eligible State
3 employees incentives for participation aimed at preventing,
4 diagnosing, treating, and managing diabetes.
5     The program may include, but is not limited to:
6         (1) Coverage for health screenings, health education,
7     body mass index (BMI) monitoring, fitness screening,
8     weight loss, and nutritional counseling.
9         (2) Coverage of all treatments approved by the Food and
10     Drug Administration (FDA) for diabetes, weight loss, and
11     obesity.
12         (3) Waiver or reduction of co-pays for disease-related
13     visits and medications, including preventive visits and
14     treatments.
15     The program shall be funded through appropriations from the
16 Diabetes Research Checkoff Fund.
17     The Department shall report to the Governor and the General
18 Assembly at the end of each year of the program on its impact
19 on patients enrolled in the program.
20     The Department shall adopt rules for the implementation of
21 this Section
 
22     Section 99. Effective date. This Act takes effect upon
23 becoming law.".
