1     AN ACT concerning license plates.
 
2     Be it enacted by the People of the State of Illinois,
3 represented in the General Assembly:
 
4     Section 5. The Illinois Vehicle Code is amended by changing
5 Section 3-655 as follows:
 
6     (625 ILCS 5/3-655)
7     Sec. 3-655. Hospice license plates.
8     (a) The Secretary, upon receipt of an application made in
9 the form prescribed by the Secretary, may issue special
10 registration plates designated as Hospice license plates. The
11 special plates issued under this Section shall be affixed only
12 to passenger vehicles of the first division and motor vehicles
13 of the second division weighing not more than 8,000 pounds.
14 Plates issued under this Section shall expire according to the
15 multi-year procedure established by Section 3-414.1 of this
16 Code.
17     (b) The color of the plates is wholly within the discretion
18 of the Secretary. The design of the plates shall include the
19 word "Hospice" above drawings of two lilies and a butterfly.
20 The Secretary may allow the plates to be issued as vanity
21 plates or personalized under Section 3-405.1 of the Code. The
22 Secretary shall prescribe stickers or decals as provided under
23 Section 3-412 of this Code.

1     (c) An applicant for the special plate shall be charged a
2 $25 fee for original issuance in addition to the appropriate
3 registration fee. Of this fee, $10 shall be deposited into the
4 Hospice Fund and $15 shall be deposited into the Secretary of
5 State Special License Plate Fund, to be used by the Secretary
6 to help defray the administrative processing costs.
7     For each registration renewal period, a $25 fee, in
8 addition to the appropriate registration fee, shall be charged.
9 Of this fee, $23 shall be deposited into the Hospice Fund and
10 $2 shall be deposited into the Secretary of State Special
11 License Plate Fund.
12     (d) The Hospice Fund is created as a special fund in the
13 State treasury. All money in the Hospice Fund shall be paid,
14 subject to appropriation by the General Assembly and approval
15 by the Secretary, as grants to a statewide organization whose
16 primary membership consists of hospice programs to be used as
17 follows:
18         (1) To fund projects relating to hospice care for
19     special groups, such as children, veterans, and ethnic,
20     religious, gender, or minority groups, or to provide
21     disease-specific research or outreach.
22         (2) To fund education and outreach for hospice
23     volunteers, patients, families, and health care
24     professionals.
25         (3) To fund informational and educational media
26     programs regarding the availability of hospice services.

1         (4) To fund the expansion or enhancement of a statewide
2     organization whose primary membership consists of hospice
3     programs toll-free referral line operated to provide
4     hospice information.
5         (5) To fund the expansion or enhancement of a statewide
6     organization whose primary membership consists of hospice
7     programs Internet website.
8         (6) To cover reasonable costs for special plate
9     advertising.
10     A statewide organization whose primary membership consists
11 of hospice programs shall distribute grant moneys received
12 under this subsection (d) through a standing committee that
13 reviews funding solicitations and awards to the Department of
14 Public Health for distribution as grants for hospice services
15 as defined in the Hospice Program Licensing Act. The Director
16 of Public Health shall adopt rules for the distribution of
17 these grants.
