AMENDMENT TO HOUSE BILL 4696

2    AMENDMENT NO. ______. Amend House Bill 4696 by replacing
3everything after the enacting clause with the following:
 
4    "Section 5. The Department of Natural Resources Act is
5amended by changing Section 1-17 as follows:
 
6    (20 ILCS 801/1-17)
7    Sec. 1-17. Licenses; privacy protection.
8    (a) For purposes of this Section, "license" means a
9license required under Article 3 of the Wildlife Code or under
10Article 20 of the Fish and Aquatic Life Code.
11    (b) As soon as practicable, the Department must assign a
12customer identification number to each applicant for a
13license. After the applicant has been assigned a customer
14identification number, the applicant may use that customer
15identification number in place of his or her social security
16number on any subsequent application for a license. The

1Department must keep a record of the social security number of
2each applicant. The Department shall notify the applicant that
3his or her social security number is kept on file with the
4Department.
5    (c) A licensee's social security number shall not appear
6on the face of his or her license.
7    (d) The Department shall offer to each applicant for a
8hunting license issued under the Wildlife Code the opportunity
9to have his or her name included in the First Person Consent
10organ and tissue donor registry maintained by the Secretary of
11State under Section 6-117 of the Illinois Vehicle Code. The
12Department must advise the applicant that he or she is under no
13compulsion to have his or her name included in the registry. An
14individual who agrees to have his or her name included in the
15First Person Consent organ and tissue donor registry has given
16full legal consent to the donation of any of his or her organs
17or tissue upon his or her death. A brochure explaining this
18method of executing an anatomical gift must be given to each
19applicant for a hunting license issued under the Wildlife
20Code. The brochure must advise the applicant (i) that he or she
21is under no compulsion to have his or her name included in this
22registry; and (ii) that he or she may wish to consult with
23family, friends, or clergy before doing so. The Department is
24authorized to collect and compile the names of individuals
25that agree to be included in the First Person Consent organ and
26tissue donor registry and provide the information to the

1Secretary of State for notation of the agreement in the First
2Person Consent organ and tissue donor registry. A hunting
3license issued under the Wildlife Code may contain information
4indicating whether the holder of the license is registered as
5a donor in the First Person Consent organ and tissue donor
6registry.
