1    AN ACT concerning health.
 
2    Be it enacted by the People of the State of Illinois,
3represented in the General Assembly:
 
4    Section 5. The Vital Records Act is amended by changing
5Section 11 as follows:
 
6    (410 ILCS 535/11)  (from Ch. 111 1/2, par. 73-11)
7    Sec. 11. Information required on forms.
8    (a) The form of certificates, reports, and other returns
9required by this Act or by regulations adopted under this Act
10shall include as a minimum the items recommended by the federal
11agency responsible for national vital statistics, subject to
12approval of and modification by the Department. All forms shall
13be prescribed and furnished by the State Registrar of Vital
14Records.
15    (b) On and after the effective date of this amendatory Act
16of 1983, all forms used to collect information under this Act
17which request information concerning the race or ethnicity of
18an individual by providing spaces for the designation of that
19individual as "white" or "black", or the semantic equivalent
20thereof, shall provide an additional space for a designation as
21"Hispanic".
22    (c) Effective November 1, 1990, the social security numbers
23of the mother and father shall be collected at the time of the

1birth of the child. These numbers shall not be recorded on the
2certificate of live birth. The numbers may be used only for
3those purposes allowed by Federal law.
4    (d) The social security number of a person who has died
5shall be entered on the death certificate; however, failure to
6enter the social security number of the person who has died on
7the death certificate does not invalidate the death
8certificate.
9    (e) If the place of disposition of a dead human body or
10cremated remains is in a cemetery, the burial permit shall
11include the place of disposition. The place of disposition
12shall include the lot, block, section, and plot or niche, and
13depth, if applicable, where the dead human body or cremated
14remains are located. This subsection does not apply to cremated
15remains scattered in a cemetery.
16    (f) The death certificate for an individual with a history
17of military service may include or may be amended to include
18the deceased individual's veteran status, the branch of the
19military that he or she served in, and the period of time that
20he or she served in the military.

22    Section 99. Effective date. This Act takes effect upon
23becoming law.
