AMENDMENT TO HOUSE BILL 5463

2    AMENDMENT NO. ______. Amend House Bill 5463 by replacing
3everything after the enacting clause with the following:
 
4    "Section 5. The Mental Health and Developmental
5Disabilities Administrative Act is amended by changing Section
653 as follows:
 
7    (20 ILCS 1705/53)  (from Ch. 91 1/2, par. 100-53)
8    Sec. 53. The Department shall create a consistent case
9coordination system for persons with a developmental
10disability who receive services provided or funded by the
11Department. The objectives of this system shall be to ensure
12that a full range of an individual's needs is identified and
13assessed through statewide use of an individual client
14assessment tool; to ensure that each individual actually
15receives, in the most effective and efficient combination and
16sequence, the full range and continuum of services needed; to

1ensure that all available resources are applied appropriately
2to each individual served; and to provide a systematic
3procedure for serving individuals which generates among and
4within the local service delivery agencies information
5required for effective system management.
6    Each individual residing in a community integrated living
7arrangement shall receive an annual assessment to screen that
8individual for any health issues or risks. Beginning July 1,
92019, each individual shall receive his or her annual client
10assessment via a web-based, electronic screening tool. The
11electronic screening tool shall replace the current
12paper-based assessment. A provider may make a request, along
13with justifications, to the Department to complete the
14assessment on paper. Subject to appropriation, the Department
15may contract with a third-party entity to create and implement
16the web-based, electronic screening tool. The Department shall
17make changes to its rules in the Illinois Administrative Code
18to incorporate a web-based, electronic assessment tool.
19    "Case coordination" means a mechanism for linking and
20coordinating segments of the service delivery system to ensure
21the most comprehensive program for meeting an individual
22client's needs. It facilitates client movement through an array
23of services so that at any given time, services received are
24matched to the needs of the individual.
