1    AN ACT concerning State government.
 
2    Be it enacted by the People of the State of Illinois,
3represented in the General Assembly:
 
4    Section 5. The Mental Health and Developmental
5Disabilities Administrative Act is amended by changing Section
653 as follows:
 
7    (20 ILCS 1705/53)  (from Ch. 91 1/2, par. 100-53)
8    Sec. 53. The Department shall create a consistent case
9coordination system for persons with a developmental
10disability who receive services provided or funded by the
11Department. The objectives of this system shall be to ensure
12that a full range of an individual's needs is identified and
13assessed through statewide use of an individual client
14assessment tool; to ensure that each individual actually
15receives, in the most effective and efficient combination and
16sequence, the full range and continuum of services needed; to
17ensure that all available resources are applied appropriately
18to each individual served; and to provide a systematic
19procedure for serving individuals which generates among and
20within the local service delivery agencies information
21required for effective system management.
22    Specific to the individual client assessment tool, each
23individual living in a community integrated living arrangement

1shall receive an annual client assessment. The annual client
2assessment shall be completed using the Health Risk Screening
3Tool. Beginning January 1, 2019, each individual shall receive
4his or her annual client assessment via the web-based Health
5Risk Screening Tool. The Health Risk Screening Tool shall
6replace the current paper-based version of the assessment. The
7Department shall make changes to the Illinois Administrative
8Code to correspond with this online version of the Health Risk
9Screening Tool.
10    "Case coordination" means a mechanism for linking and
11coordinating segments of the service delivery system to ensure
12the most comprehensive program for meeting an individual
13client's needs. It facilitates client movement through an array
14of services so that at any given time, services received are
15matched to the needs of the individual.

17    Section 99. Effective date. This Act takes effect upon
18becoming law.
