AMENDMENT TO SENATE BILL 1410

2    AMENDMENT NO. ______. Amend Senate Bill 1410, AS AMENDED,
3with reference to page and line numbers of Senate Amendment No.
41, on page 11, line 26, after "State." by inserting "However,
5the health care provider's signature on the certificate
6reflects only that education was provided and does not allow a
7health care provider grounds to determine a religious
8exemption. Those receiving immunizations required under this
9Code shall be provided with the relevant vaccine information
10statements that are required to be disseminated by the federal
11National Childhood Vaccine Injury Act of 1986, which may
12contain information on circumstances when a vaccine should not
13be administered, prior to administering a vaccine. A healthcare
14provider may consider including without limitation the
15nationally accepted recommendations from federal agencies such
16as the Advisory Committee on Immunization Practices, the
17information outlined in the relevant vaccine information
18statement, and vaccine package inserts, along with the

1healthcare provider's clinical judgment, to determine whether
2any child may be more susceptible to experiencing an adverse
3vaccine reaction than the general population, and if so, the
4healthcare provider may exempt the child from an immunization
5or adopt an individualized immunization schedule."; and
 
6on page 12, lines 5, by replacing "annually" with "prior to
7entering kindergarten, sixth grade, and ninth grade".
