HOUSE RESOLUTION

 
2    WHEREAS, The members of the Illinois House of
3Representatives wish to congratulate Bishop Horace E. Smith,
4M.D. on the occasion of his 35th anniversary with Apostolic
5Faith Church; and
 
6    WHEREAS, Bishop Smith was born and raised on the South Side
7of Chicago; he graduated from Lindblom High School and received
8his Bachelor of Science from Chicago State University in 1971
9and his Doctor of Medicine from the University of Illinois
10Medical Center in 1975; he completed his Pediatric Residency
11with the University of Illinois Hospital in 1978 and his
12Clinical Fellowship in Pediatric Hematology/Oncology with the
13Children's Memorial Hospital at Northwestern University in
141980; and
 
15    WHEREAS, On March 28, 1980, Bishop Smith became Pastor of
16the Apostolic Faith Church; under his leadership, the
17congregation has grown from 200 members to 7,000; each year his
18ministry provides assistance by way of food and supplies to
19over 4,000 homeless families around the Chicago area, a
20scholarship program that grants more than $100,000 each year,
21visitations to nursing homes, outreach to correctional
22facilities, and a JobSource program, which to date has assisted
23more than 1,000 individuals in obtaining employment; and

1    WHEREAS, In August of 2004, Bishop Smith was elevated to
2the office of Presiding Bishop of the Pentecostal Assemblies of
3the World, Inc.; the P.A.W. has over 1.5 million members
4worldwide, including in Africa, Asia, Europe, Australia, and
5New Zealand; his humanitarian endeavors reach beyond the shores
6of the United States to support missions in all parts of Africa
7through his partnership with World Vision USA, where he serves
8as a board member; under his leadership, Apostolic Faith Church
9provides resources for the drilling of wells and they provide
10food, clothing, education, medical assistance, and other goods
11and services; over 2,000 children have been sponsored by the
12church through World Vision USA; and
 
13    WHEREAS, Bishop Smith has served as Director of the
14Comprehensive Sickle Cell/Thalassemia Program at Children's
15Memorial Hospital for over 20 years; he is an Assistant
16Professor of Pediatrics at the Northwestern Feinberg School of
17Medicine and is recognized world-wide as an outstanding
18pediatrician; he has been honored by numerous organizations,
19including the Children's Miracle Network, and has been featured
20in local and national newspapers, magazines, and on television;
21he currently serves on the boards of the historic Wabash YMCA,
22the Family Institute at Northwestern University, and the
23University of Chicago Cancer Research Center-Community Health
24Advisory Board; and

1    WHEREAS, Bishop Smith has been happily married to Susan
2(nee Davenport) since 1976; they have 3 daughters, Lauren
3(Brian) Elrod, Rachel (Courtney) Horton, and Emily (Patrick)
4Green and 4 grandchildren; therefore, be it
 
5    RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE
6NINETY-NINTH GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that we
7congratulate Bishop Horace E. Smith, M.D. on his 35th
8anniversary with Apostolic Faith Church and wish him many more
9wonderful years; and be it further
 
10    RESOLVED, That a suitable copy of this resolution be
11presented to Bishop Smith as an expression of our esteem and
12respect.
