2 | | WHEREAS, The members of the Illinois House of |
3 | | Representatives are pleased to honor Bishop Horace E. Smith on |
4 | | his 32nd anniversary as Pastor of Apostolic Faith Church in |
5 | | Chicago; and
|
6 | | WHEREAS, He graduated as an honor student from the Lindblom |
7 | | High School in Chicago, received his Bachelor of Science (with |
8 | | honors) degree from Chicago State University; his M.D. from the |
9 | | University of Illinois Medical Center; completed his Pediatric |
10 | | Residency at the University of Illinois Hospital; and completed |
11 | | his Clinical Fellowship in Pediatric Hematology/Oncology at |
12 | | Children's Memorial Hospital/Northwestern University; and
|
13 | | WHEREAS, He served as director of the Comprehensive Sickle |
14 | | Cell/Thalassemia Program at Children's Memorial Hospital; he |
15 | | currently serves as Assistant Professor of Pediatrics at |
16 | | Northwestern Medical School, and is recognized world-wide as an |
17 | | outstanding pediatrician in his field; and
|
18 | | WHEREAS, He became pastor of the Apostolic Faith Church on |
19 | | March 28, 1980; he was appointed a District Elder of the |
20 | | Illinois District Council (I.D.C.) of the Pentecostal |
21 | | Assemblies of the World in 1983; he served as Chairman of the |
22 | | I.D.C. from 1986-1991; he was appointed to the Board of Aenon |

1 | | Bible College of the Pentecostal Assemblies of the World in |
2 | | 1989; and he was appointed a Suffragan Bishop in 1994; and
|
3 | | WHEREAS, He served as the General Secretary of the |
4 | | Pentecostal Assemblies of the World 1996-1997, and was elevated |
5 | | to the office of Bishop in August of 1997; in June 1998, Dr. |
6 | | Smith was installed as Diocesan Bishop of the 14th Episcopal |
7 | | District of the Pentecostal Assemblies of the World, which |
8 | | includes the states of Kansas, Colorado, and Western Missouri; |
9 | | on July 29, 2004, Bishop Smith was elected to the office of |
10 | | Presiding Bishop of the Pentecostal Assemblies of the World and |
11 | | served 2 full terms until August of 2010; and |
12 | | WHEREAS, Dr. Smith serves on the Board of the historic |
13 | | Wabash YMCA, which was purchased by 4 community churches, |
14 | | including Apostolic Faith Church; the churches redeveloped and |
15 | | reshaped the Y for community services, including SRO housing; |
16 | | since 2001, Dr. Smith has also served as a Board Member of the |
17 | | Family Institute at Northwestern University, as a member of the |
18 | | University of Chicago Cancer Research Center-Community Health |
19 | | Advisory Board, and more recently, as a member of World Vision |
20 | | International; and |
21 | | WHEREAS, In 2002, Dr. Smith and the Apostolic Faith Church |
22 | | partnered with World Vision to sponsor orphaned children whose |
23 | | parents died as a result of HIV/AIDS; to date, over 1,000 |

1 | | children have been adopted by members of the congregation; Dr. |
2 | | and Sis. Smith have traveled to South Africa, along with World |
3 | | Vision, to view first-hand the devastation caused by the AIDS |
4 | | virus; and
|
5 | | WHEREAS, Dr. Smith is an avid and accomplished Bible |
6 | | scholar and has been part of many organizations within the |
7 | | church, from the Boy Scout Troop of his youth to singing in the |
8 | | choirs; his dedicated ministry reaches not only the |
9 | | congregation but his belief in God and the power of salvation |
10 | | and healing extends to his medical profession as a concerned |
11 | | and committed pediatrician (specializing in |
12 | | hematology/oncology) for critically ill children; through his |
13 | | prayers and faith in God as a healer, Dr. Smith has seen God's |
14 | | healing power reverse the course of illness in the lives of his |
15 | | patients;
and |

17 | | WHEREAS, Bishop Smith has been happily married to Susan |
18 | | Davenport Smith since 1976; they met at the University of |
19 | | Illinois where Susan graduated with a B.S. in Pharmacy; they |
20 | | have 3 beautiful daughters, Lauren
Elrod, Rachel Horton, and |
21 | | Emily Green, along with their sons-in-law, Brian Elrod, |
22 | | Courtney Horton, and Patrick Green; and granddaughter, |
23 | | Lillian; therefore, be it
