| 2 | | Be it enacted by the People of the State of Illinois,

| 3 | | represented in the General Assembly:

| 4 | | Section 5. The Mental Health and Developmental |
| 5 | | Disabilities Code is amended by changing Sections 3-201 and |
| 6 | | 4-202 as follows:

| 8 | | Sec. 3-201.
The Department shall prescribe all forms |
| 9 | | necessary for
proceedings under this Chapter, and all forms |
| 10 | | used in such proceedings shall
comply substantially with the |
| 11 | | forms so prescribed. The Department shall
publish all forms in |
| 12 | | electronic format and post the forms to its website print and |
| 13 | | furnish an initial supply of such forms to the clerks of the |
| 14 | | circuit courts.
