1    AN ACT concerning State government.
 
2    Be it enacted by the People of the State of Illinois,
3represented in the General Assembly:
 
4    Section 5. The Department of Public Health Powers and
5Duties Law of the Civil Administrative Code of Illinois is
6amended by changing Section 2310-600 as follows:
 
7    (20 ILCS 2310/2310-600)
8    Sec. 2310-600. Advance directive information.
9    (a) The Department of Public Health shall prepare and
10publish the summary of advance directives law in Illinois that
11is required by the federal Patient Self-Determination Act.
12Publication may be limited to the World Wide Web.
13    (b) The Department of Public Health shall adopt, by rule,
14and publish Spanish language versions of the following:
15        (1) The statutory Living Will Declaration form.
16        (2) The Illinois Statutory Short Form Power of Attorney
17    for Health Care.
18        (3) The statutory Declaration of Mental Health
19    Treatment Form.
20        (4) The summary of advance directives law in Illinois.
21        (5) The Department of Public Health Uniform DNR Advance
22    Directive Any statewide uniform Do Not Resuscitate forms.
23    Publication may be limited to the World Wide Web.

1    (b-5) In consultation with a statewide professional
2organization representing physicians licensed to practice
3medicine in all its branches, statewide organizations
4representing nursing homes, registered professional nurses,
5and emergency medical systems, and a statewide organization
6representing hospitals, the Department of Public Health shall
7develop and publish a uniform form for physician
8do-not-resuscitate orders that may be utilized in all settings.
9The form shall meet the minimum requirements to nationally be
10considered a physician orders for life-sustaining treatment
11form, or POLST, and may be referred to as the Department of
12Public Health Uniform DNR Advance Directive. This advance
13directive does not replace a physician's do-not-resuscitate
14(DNR) order.
15    (c) (Blank). The Department of Public Health may contract
16with statewide professional organizations representing
17physicians licensed to practice medicine in all its branches to
18prepare and publish materials required by this Section. The
19Department of Public Health may consult with a statewide
20organization representing registered professional nurses on
21preparing materials required by this Section.
