Synopsis As Introduced
Amends the Mental Health and Developmental Disabilities Code. Repeals provisions requiring the Department of Human Services to prescribe all forms necessary for proceedings concerning (i) the admittance of a person as an inpatient to a mental health facility for treatment of mental illness and (ii) the admittance of a person with a developmental disability to a facility for residential and habilitation services. Effective immediately.

Replaces everything after the enacting clause. Amends the Mental Health and Developmental Disabilities Code. Provides that the Department of Human Services shall publish all forms concerning admission to mental health and developmental disability facilities in electronic format and post the forms to its website (rather than print and furnish an initial supply of such forms to the clerks of the circuit courts). Effective immediately.
