| HOUSE RESOLUTION

| WHEREAS, The members of the Illinois House of |
3 |
| Representatives are pleased to congratulate Dr. Jim Czerwionka |
4 |
| of Good Shepherd Manor for a job well done; and
|
5 |
| WHEREAS, In 1993, the GSM Board picked Dr. Jim Czerwionka |
6 |
| from a group of highly qualified candidates to lead Good |
7 |
| Shepherd in what was to become the largest sustained building, |
8 |
| staffing, and major fund raising efforts in the Manor's |
9 |
| history; through his leadership two Intermediate Care |
10 |
| Facilities, 13 CILAs, and a state of the art infirmary have all |
11 |
| been added to the facilities; and
|
12 |
| WHEREAS, Dr. Czerwionka led the construction and |
13 |
| acquisition of fourteen group homes to provide individualized, |
14 |
| community based treatment; he worked with State and local |
15 |
| government to gain a water and sewer grant of $208,000; he led |
16 |
| the development of a community-based sheltered workshop as well |
17 |
| as employment in the community for residents; he led the charge |
18 |
| in identifying and addressing the rapidly increasing needs of |
19 |
| an aging resident population through the construction of a |
20 |
| specially designed Alzheimer's Group Home and a 7,000 square |
21 |
| foot Infirmary; he increased the Manor's operational budget |
22 |
| from $2 million to $7 million and increased the staff from 60 |
23 |
| to 170; he maintained and supported appeals and fundraising |

| activities to generate over $1 million annually; and he |
2 |
| established the Shepherds Endowment Society; and
|
3 |
| WHEREAS, Prior to coming to Good Shepherd Manor, Jim |
4 |
| Czerwionka had an admirable career history with a long list of |
5 |
| achievements in treatment centers, hospital administration, |
6 |
| higher education, and professional associations; he has held |
7 |
| board positions and leadership roles, worked aggressively in |
8 |
| program development and planning, guided large scale building |
9 |
| and renovation programs, led regulatory compliance and quality |
10 |
| control initiatives, and has been an active leader in public |
11 |
| relations, fundraising, research, staff development and |
12 |
| training, and fiscal and facilities management; and
|
13 |
| WHEREAS, Dr. Czerwionka was the executive director for the |
14 |
| Larkin Center for Children and Adolescents in Elgin, director |
15 |
| for the Nachusa Lutheran Home for Children, and a department |
16 |
| head at Pritzker Children's Hospital in Chicago; he holds a |
17 |
| Doctoral Degree in Psychology from Northern Illinois |
18 |
| University, a Masters of Education in Special Education and |
19 |
| Rehab Counseling from the University of Pittsburgh, a Master of |
20 |
| Science in Child Development and Child Care from the University |
21 |
| of Pittsburgh, and a Bachelors in Philosophy/Education from St. |
22 |
| Mary of the Lake Seminary; and |
23 |
| WHEREAS, He has held faculty positions at a number of |

| faculty positions including the Arthur J. Audy Detention Center |

| in Chicago; College of St. Francis, Joliet; Sauk Valley |
3 |
| College, Dixon; Roosevelt University, Chicago; and at Olivet |
4 |
| Nazarene University in Bourbonnais; he has also been an active |
5 |
| community leader over the years, playing key committee and |
6 |
| leadership parts in a number of organizations including the |
7 |
| Lions, Knights of Columbus, and currently, the |
8 |
| Bradley/Bourbonnais Rotary Club, where he has held roles |
9 |
| including Youth Exchange Officer, International Chair, Club |
10 |
| President, and Foundation Chair; and
|
11 |
| WHEREAS, His wife, Marcia, is a school teacher; they are |
12 |
| parents to three children, Bridgette, John, and Lori, and the |
13 |
| grandparents to two grandchildren with a third to arrive very |
14 |
| soon; Jim grew up in Chicago and was enrolled in the Catholic |
15 |
| seminary system before transferring to a career in social |
16 |
| ministry; and |
17 |
| WHEREAS, In addition, Jim Czerwionka has held an active |
18 |
| leadership role with the Rotary District #6450 where he has |
19 |
| worked actively with the group study exchange program at the |
20 |
| district level; he is currently the district assistant |
21 |
| governor, and will assume the role of district governor for the |
22 |
| 2009-2010 term; therefore, be it
