1 | AN ACT concerning health.
| ||||||
2 | Be it enacted by the People of the State of Illinois,
| ||||||
3 | represented in the General Assembly:
| ||||||
4 | Section 1. Short title. This Act may be cited as the Health | ||||||
5 | Access Network Act. | ||||||
6 | Section 5. Purpose. The purpose of this Act is to provide | ||||||
7 | aid to existing primary health care safety nets for specialty | ||||||
8 | care that will allow the development of a comprehensive system | ||||||
9 | of health care services. | ||||||
10 | Section 10. Definition. In this Act, "Department" means the | ||||||
11 | Department of Public Health. | ||||||
12 | Section 15. Pilot program. The Department shall establish a | ||||||
13 | Health Access Network pilot program in Kane County. The program | ||||||
14 | shall provide funding for and coordination of health care | ||||||
15 | services that include comprehensive case management and | ||||||
16 | ancillary services to support treatment goals. In particular, | ||||||
17 | the program shall provide specialty health care services, | ||||||
18 | pharmaceutical drug assistance, and diagnostic testing for | ||||||
19 | individuals who are not eligible for medical assistance under | ||||||
20 | the Illinois Public Aid Code and who do not have other health | ||||||
21 | insurance coverage. The program shall emphasize providing |

1 | services in relation to the top health care issues as | ||||||
2 | identified by Kane County's "IPLAN" community health | ||||||
3 | assessment and planning process and the Top Ten Diseases as | ||||||
4 | determined by the Centers for Disease Control and Prevention. | ||||||
5 | Section 20. Lead agency; grant. | ||||||
6 | (a) The Department shall designate an entity to serve as | ||||||
7 | the lead agency in administering the pilot program. Any entity | ||||||
8 | located in Kane County that has established consortia of health | ||||||
9 | entities may apply for designation as the lead agency and for a | ||||||
10 | grant under this Section. The Department shall determine the | ||||||
11 | form and manner of application. The application must state the | ||||||
12 | name and address of the proposed lead agency. | ||||||
13 | (b) Any entity, or a group of individuals or entities or a | ||||||
14 | combination of both, located in a county with a population of | ||||||
15 | less than 100,000 that is contiguous to Kane County may join | ||||||
16 | with an entity described in subsection (a) in applying for a | ||||||
17 | grant under this Section to establish the pilot program to | ||||||
18 | serve a multi-county area that includes Kane County. The lead | ||||||
19 | agency for such a multi-county program must be located in Kane | ||||||
20 | County, however. | ||||||
21 | (c) The Department shall award a grant to the lead agency | ||||||
22 | designated under subsection (a) for the purpose of | ||||||
23 | administering the pilot program. The lead agency shall use the | ||||||
24 | grant moneys to administer and coordinate pilot program | ||||||
25 | activities within the county or counties of the program's |

1 | operation and shall develop community partnerships necessary | ||||||
2 | to implement the pilot program. | ||||||
3 | Section 25. Report. The Department, in cooperation with the | ||||||
4 | lead agency designated under Section 20, shall submit a report | ||||||
5 | on the operation of the pilot program to the General Assembly | ||||||
6 | after the pilot program has been in operation for 18 months. | ||||||
7 | The report shall include the Department's recommendations | ||||||
8 | concerning the continued operation of the Health Access Network | ||||||
9 | program and its expansion to other areas of the State. | ||||||
10 | Section 30. Implementation subject to appropriations. | ||||||
11 | Implementation of this Act is subject to appropriations made to | ||||||
12 | the Department for that purpose.

13 | Section 99. Effective date. This Act takes effect upon | ||||||
14 | becoming law.
