| institution, laboratory or
diagnostic

| dispensary, ambulatory surgical

| facility. "Health care facility" does not include a physician's |

| or health care practitioner's office."; and

| on page 4, line 25, by replacing "medical" with "professional |
| 11 |
| or graduate"; and
|
| 12 |
| on page 5, line 2, by deleting "medical or other"; and

| 13 |
| on page 5, line 8, by deleting "medical school or".
