| HOUSE RESOLUTION
|
2 |
| WHEREAS, Hazel J. Lewis has served the State of Illinois |
3 |
| and the nation continuously as Executive Director of the |
4 |
| American Medical Association Alliance, Inc. since July 1, 1973; |
5 |
| and
|
6 |
| WHEREAS, In 52 years of service, she has been a |
7 |
| distinguished leader, motivator, and innovator of programs |
8 |
| benefiting all of America; and
|
9 |
| WHEREAS, Hazel has led highly successful volunteer group |
10 |
| efforts attacking the sources of violence among women and |
11 |
| children that saved lives and improved conditions for hundreds |
12 |
| of thousands of Americans; and
|
13 |
| WHEREAS, She will continue serving the citizens of Illinois |
14 |
| in local and statewide philanthropic and professional |
15 |
| organizations helping children, the elderly, and those with |
16 |
| chronic disease; and
|
17 |
| WHEREAS, The American Medical Association Alliance honors |
18 |
| Helen J. Lewis for her 52 years of service upon the occasion of |
19 |
| her retirement on April 8, 2005; therefore, be it
|
20 |
| RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE |
21 |
| NINETY-FOURTH GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that |
22 |
| we commemorate the work of Hazel J. Lewis by designating April |
23 |
| 8, 2005 to be Hazel J. Lewis Day in Illinois and we encourage |
24 |
| all in our State to be aware of her important health advocacy |
25 |
| work; and be it further
|
26 |
| RESOLVED, That a suitable copy of this resolution be |
27 |
| presented to Hazel J. Lewis as an expression of our respect and |
28 |
| esteem.
