| WHEREAS, The members of the House of Representatives of the |
3 |
| State of Illinois are pleased to congratulate Gwendolyn Mastin |
4 |
| on being named a recipient of the Robert Wood Johnson Community |
5 |
| Health Leadership Program award, the nation's highest honor for |
6 |
| community health leadership; and
|
7 |
| WHEREAS, In 1991, Ms. Mastin founded New Phoenix Assistance |
8 |
| Center, Chicago's first scattered-site housing program for |
9 |
| homeless women infected with HIV or AIDS and their children; |
10 |
| and
|
11 |
| WHEREAS, New Phoenix's scattered-site housing program |
12 |
| promotes independent permanent living situations for |
13 |
| participants that are homeless and may be infected or affected |
14 |
| by the HIV/AIDS virus within the South Shore community; the |
15 |
| agency also provides transitional housing for pregnant teens |
16 |
| and young mothers who are wards of the State, homeless women, |
17 |
| and homeless families; since it was founded, the program has |
18 |
| served more than 5,700 people in the metropolitan Chicago area; |
19 |
| and
|
20 |
| WHEREAS, Ms. Mastin chairs the State Advisory Committee on |
21 |
| Child Abuse and Neglect, is the co-founder of X-Mas in April |
22 |
| Metro Chicago, serves on the University of Illinois at Chicago |
23 |
| Extension Committee, and is an active participant in the |
24 |
| Chicago Continuum of Care; and
|
25 |
| WHEREAS, The Robert Wood Johnson Community Health |
26 |
| Leadership Program (CHLP) awards $1.2 million each year to |
27 |
| individuals who have overcome significant challenges to expand |
28 |
| access to health care and social services to underserved |
29 |
| members of their communities; Ms. Mastin was chosen from nearly |
30 |
| 800 nominations and is one of 10 winners of the national award |
31 |
| honored in 2004; the award comes with $105,000 to enhance the |
